• Create Account: Register and submit a $250 application fee to be considered for 4 or 5 star status.
• Data Submission: Data will be accepted quarterly for review and received via a secure portal.
• Required Data: Submit a rolling year report from your survey provider, including top box and percentile rankings. Data submission windows are posted on the upload data page.
• Include: Domain totals for cleanliness and quietness, hospital rating, and recommendation.
• Verification: Verify the number of surveys returned to meet minimum requirements.
